VOLUNTEER REGISTRATION FORM

As a volunteer of Safe Pet Project (SPP), I agree to support the organization’s Mission Statement (as printed below) as well the purpose and policies stablished by Safe Pet Project’s board of directors. I understand that as representative of SPP, if I at any time knowingly act in conflict with the mission statement or violate the organization’s policies, I may be asked to leave as a volunteer. I understand there are certain risks involved in the organization. I accept these risks and will not hold SPP responsible for any accident or injury.
